Sometimes it is necessary for the doctor to look at your medical records to see what was written at the time of your injury and to see if you have had any previous problems that could affect how quickly you recover.
A consent form signed by you, that gives permission for the hospital or GP to send us the notes is required in order for Premex Services to obtain your relevant medical records. If you do receive one of these forms, it is important that you complete it fully and return it to Premex Services as quickly as possible.
A delay in making your appointment and completing your report may occur if Premex Services do not receive the signed consent form as soon as possible.
